Beginning July 2025, <a role=\"link\" href=\"https:\/\/unioncommunitycare.org\/\">Union Community Care<\/a> will start delivering accessible, inclusive, integrated healthcare services to meet the evolving needs of the Greater Coatesville community as part of AHE\u2019s ARPA-funded integrated health strategy.<\/p>\n<p>As the core provider in this model, Union Community Care (Union) will deliver patient-centered, tailored care that integrates primary medical care, maternal and child health, dental care, behavioral health, and social support services. \u00a0As a nationally recognized Patient-Centered Medical Home certified for quality and excellence, Union Community Care is a trusted FQHC with a proven track record of delivering its \u201c<a role=\"link\" href=\"https:\/\/unioncommunitycare.org\/our-purpose\/\">signature <em>whole health, whole community <\/em>model of healthcare<\/a>\u201d.<\/p>\n<p>\u201cWe are excited to welcome Union Community Care as a partner in our shared mission to improve health outcomes and ensure access for folks as they need it,\u201d said Kevin Ressler, President &amp; CEO of AHE. \u201cUnion\u2019s innovative approach and expertise in expanding service offerings will play a critical role in shaping the future of healthcare in Greater Coatesville. Union\u2019s dedicated Care Teams will reach out to ChesPenn patients starting in June, offering personalized support and coordinated care to ensure continuity and trust.
